WHEN SCHEDULING AN EXAM, PLEASE HAVE THE FOLLOWING INFO AVAILABLE FOR RECEPTIONIST:

• Patient name, telephone number, type of test, type of insurance, referring doctor’s full name, phone and fax number.
• After booking the appointment, please fax insurance information and prescription immediately so that insurance certification can take place.
• Please be aware all examinations require a written order from the physician with a diagnosis, if we do not have the above information there will be a delay in the patients exam.
• Workman’s Comp: fax claim # and date of accident, name of insurance, name & phone number of adjuster.
• Lien Case: if available, fax attorney info. & police report with insurance & policy number, with claim #.
• When scheduling an appointment, please inform our receptionist if the patient needs transportation. Give address and phone number of patient.
• At the time of scheduling, please inform us if the patient needs their films or if the doctor would like films delivered to your office.
• Please remember patients results are available 24 hours after the time of their completed test.
• Verbal reports are given for emergency cases if requested.

Billing Policy:

• Payment in full will be required at time of service unless prior arrangements have been made.
• MRI Lincoln Imaging Center will file for insurance. However, patient is charged for deductible and/or co-payment accordingly to their plan.
• All HMO patients must bring a referral for the scheduled exam.
• We accept all major credit cards (American Express, Visa, Master Card) cash, cashiers check & certified check. (no personal checks)
